Recertification Credits on CD-ROM

Now you can obtain recertification credits in the comfort of your own home/office in 3 easy steps…

1. View the program and take the quiz.
2. Fill out the certification form.
3. Mail the completed quiz and certification form to us.

After you have completed viewing the program, you will complete an exam. If your score is 70% or more, you will print out your quiz results. Mail it back to us in the envelope we included in your packet, along with your registration # & certification form. Your recertification certificate will be mailed to you in about 2 weeks.

We are currently offering the following CD’s… If you purchased the CD-Rom’s in 2004, you may re-use them to acquire credits again during 2005. The next version of the CD-Rom’s are not ready yet. We have extended the time period in which you are able to acquire credits. Before using the CD-Rom’s again in 2005, please contact Alysia (845-357-7734 or alysiapmtc@optonline.net) for new registration #’s.
